Baptism cards get the wrong verses more than almost any occasion, because people reach for baby verses when the baptism is an adult’s, or adult verses when it is an infant’s. These are sorted. The first two are about baptism itself; the rest are for the child, the new believer, and the family standing up front.

Jesus comes up out of the water and a voice says, “This is my beloved Son, with whom I am well pleased.” The first baptism in the Gospels, and the words spoken over it are the words every baptism echoes: beloved, pleased. What to write with it
One line before the verse is usually enough.
For a baby: Welcome to the family, little one. We’ll be praying this over you for a long time.
For an adult: Proud of you. This is a beginning, and we’re glad to be in it with you.
For the parents: What a day. Thank you for letting us be part of it.
For godparents, to write to the child for later: I stood up for you today and promised to keep showing up. I mean it.

Galatians 3:27

“As many of you as were baptized into Christ have put on Christ.” Baptism as being clothed, which is a picture a teenager or adult can hold: something put on that changes how you are seen. Short, and it fits a card for someone who has just made the decision for themselves rather than had it made for them.

Jesus, indignant on the children’s behalf: “Allow the little children to come to me.” Then he takes them in his arms and blesses them. For an infant baptism or dedication, this is the picture of what is happening: a child brought forward, and welcomed.
Numbers 6:24-26
‘Yahweh bless you, and keep you. Yahweh make his face to shine on you, and be gracious to you. Yahweh lift up his face toward you, and give you peace.’

The blessing Aaron was told to say over the people. It is the best thing to write in a baptism or dedication card if you want to bless the child rather than describe the ceremony: keep you, shine on you, give you peace. Many churches say it over the child that day; writing it makes your card part of that.

Hannah, bringing the son she prayed for to the temple to give him back to God: “For this child I prayed.” This is the original dedication, and the verse for parents who waited, adopted, or are standing up front with a child they nearly didn’t have.
The one I’d leave out
Jeremiah 29:11. It arrives in baptism cards as a life promise and it was written to exiles about the nation’s return. Matthew 3:17, beloved, well pleased, is what baptism actually says over a person.
How to use these verses
Matthew 3:16–17 for anyone. Romans 6:4 if the person being baptized chose it themselves. Numbers 6:24–26 for a baby, written out in full. If you are a godparent, write a second card for the child to open at eighteen; families keep those.

What is the difference between a baptism and a dedication card?
A dedication is the parents presenting the child and promising to raise them in the faith; 1 Samuel 1:27 is the model. A baptism is a sacrament; Matthew 3:16–17 and Romans 6:4 are about that. Numbers 6:24–26 works for either.
